Group insurance rates are based on a simple group profile. To provide a Profile for your business, please take a few minutes to complete the Group Profile Form below.
Naturally, in order to develop a quote for you we will also need information about the group members. Please complete the Census Form below to provide a listing of the employees in your group.
In addition, some association plans require a brief health risk profile of the group Health Risk Questionnaire below.

This form is needed only for those who wish to make an application as a member of an association program or certain chamber groups (e.g.: Associated Employer Trust, Greater Vancouver Chamber, ALLtech, etc.)
